What a tough spot to be in — clear scans are wonderful news, but dealing with osteoporosis as a side effect without being able to take bone medications is genuinely challenging.

This is really a conversation to have directly with your oncologist, as stopping or adjusting letrozole is a significant decision that needs to be weighed carefully. Some things worth discussing with your doctor include:

- Reassessing the benefit vs. risk of continuing letrozole long-term given your clear scans and bone health impact
- Dose reduction as a possible middle ground rather than stopping completely
- Non-medication approaches to bone health, such as weight-bearing exercise, calcium-rich diet, and vitamin D supplementation
- A second opinion from another gynecologic oncologist about whether lifelong hormone therapy is still necessary given your current status

The decision to stop or continue letrozole should never be made alone — the risk of cancer recurrence has to be carefully balanced against the bone health risks. A second opinion could give real clarity here.
